This is a boutique telehealth practice that operates 99% virtually. I have found that telemedicine is new for most patients and that brings some initial hesitation. I have also found that after the first appointment, no one ever wants to sit in a waiting room again!
I am licensed to practice in Washington, DC and Mississippi as a Family Nurse Practitioner and Certified Nurse-Midwife. Laws regulating telemedicine are actively being written. I have hope that that one day there will be no state restrictions when accessing your care virtually, but as for now, these are the two states my patients must reside in.
I do not have a set schedule for when I am in either place, so if you would prefer an in-person appointment for your first visit, I am happy to make this happen, but there could be a several month delay. I encourage you to be curious about telemedicine for a Discovery Session. I anticipate that you will be pleasantly surprised.
I do not like to rush. It is my goal to know the whole of you, and that takes time. I block 2 hours for each appointment, which may or may not be fully used. That depends on our work together.
Within 48 hours (often sooner than that), you will receive a follow up email that comprehensively summarizes our visit. There will never be any need for you to take notes during the visit. I want you to be fully present for the work and leave the note-taking up to me.
A Discovery Session comes with a no-strings attached guarantee. We will know at the end of that appointment if this practice is right for you.
If you decide to continue under my care and purchase a 6 or 12 month package,
Respecting each other’s time creates a foundation for quality, consistent healthcare. Due to the large blocks of time dedicated, I have a 3 day cancellation or rescheduling policy.This includes business days only. Weekends do not count. This means that a Wednesday appointment needs to be canceled by the prior Friday. I reserve the right to charge $50 to your credit card if the appointment is not canceled or rescheduled before this 3 day deadline. If unforeseen acts of God or an acute emergency happens preventing you from doing this, please just let me know. I am human too, and know that these things happen!
If you have not logged into the appointment or have not arrived in person for 15 minutes after the appointment has started, I will end it and you will need to reschedule. If you are ever having any problem logging into Zoom, please call me within the first 15 minutes and let me help you troubleshoot. The number to call is 202-681-5343.
I have found that once money is no longer a part of our conversation, the Radical work we will do together has more space. With that in mind, I prefer, if possible, your entire package be paid in full. To thank you for this, there will be a 5% discount if packages are paid for in full in the beginning.
At the same time that you schedule an appointment, you will also be asked to pay. Please be mindful which package that you are choosing. There is one for a full payment and then another that will automatically bill your credit card each month.
My FEES are:
6 month package – $750/month or $4275 if paid in full (inclusive of 5% discount).
12 month package – $710/month or $8075 if paid in full (inclusive of 5% discount).
Annual follow up visits will be $650, inclusive of saliva test kit which will need to be completed 2 weeks prior to appointment.
All Concierge packages will be charged monthly.
Medical records can only be released with your authorization. It is your responsibility to obtain previous medical records from other physicians or health care providers that you wish for me to review. If you feel your medical records are pertinent to your appointment, please obtain these records as soon as you can (it often takes offices a long time to process these requests) and make sure that I have received them at least 1 week prior to your appointment.
Please have all records emailed to me at : Stephanie@theradicalwithin.com . I do not have a fax and if you have them in physical form, please scan and email to me directly. This is the only way I can receive medical records.
I do not accept health insurance and do not assist in the resolution of insurance claims. I am a Medicare provider and for those with Medicare, I will discuss with you the supplemental paperwork that will be required for your saliva testing. Upon request, I can provide you a receipt which you can submit to your insurance carrier which will include appropriate medical codes.
The preferred way to contact me is via email at email@example.com . Although the server I use is HIPPA compliant, please note that I cannot guarantee a 100% secure email system. I do not use encryption with our email communications. In agreeing to work with me, you are consenting to using regular email to send and receive your patient information.

I offer direct access to medical grade supplements for the benefit of my patients. It is often the case that significant overwhelm takes place when you walk down the supplement aisle. I have curated my own recommendations, not only of products but also brand names that I highly recommend. My Wellevate and Standard Process links are available for you to access these hand-picked formularies, however there is no obligation to purchase supplements through these links.
Electronic recording of appointments and/or conversations is strictly prohibited, unless I grant prior approval in writing to you. It is illegal and unethical to record a medical appointment without the knowledge of the medical provider. If such recording takes place, legal action may be taken.
Under my care, I can not and am not acting as your primary care physician. As a result, I do not address urgent care or primary care issues, other than basic needs for prescriptions that I feel would safely address an acute situation. I do not manage any chronic health issues such as hypertension or diabetes. I highly recommend that you have a primary care physician. If you have a serious health problem that requires immediate attention, you should call your other doctor(s), call 911, or have someone take you to the nearest hospital emergency room. If you notice an adverse effect from one of the components of your treatment, you should discontinue it and then email me.
If you have a medical emergency, call 911 or go directly to the nearest emergency room. Similarly, we do not check emails daily and therefore cannot guarantee immediate prescription refills or urgent answers.
